Why does presbyopia occur?

It is produced by a progressive aging of the lens, a structure similar to a zoom of a camera. The lens is a lens located behind the pupil and in front of the retina, which is able to change its shape according to the visual needs at all times.

From the 40 years, this lens is losing elasticity and therefore its ability to accommodate, hindering everyday tasks such as reading or using the mobile. This problem can be corrected with the use of specific glasses or contact lenses. In more advanced ages, the lens will also be opacified resulting in the development of the cataract.

  • Decreased elasticity of the substance and the lens capsule.
  • The volume of the lens increases progressively with age. It is more difficult to deform a large body than a small body.
  • Reduction of the zonulo-ciliary mechanical efficiency (muscles and tendons of focus), due to changes in its geometry.

The main symptom is the poor near vision that manifests when the patient begins to move away from nearby objects, in order to obtain a clear image. This can also lead to eyestrain or eyestrain, presenting red eye, stinging, eye heaviness, headache ... Other symptoms are also highlighted:

  • Reverse of the next point, which forces the reading material away to be seen more clearly.
  • In the incipient presbytes, in relation to the attempts of excessive accommodative effort, it is even possible to produce a pseudomyopia (focus spasm).
  • Also due to the increase in accommodative effort and the synkinetic reflection of the accommodative convergence that it carries with it, a transient diplopia can be caused.

To date, there is no preventive or curative treatment capable of completely restoring the properties of the natural lens, but there are different surgical options that help you see at different distances without the need to use glasses again.

The most effective is the replacement of the crystalline lens with multifocal intraocular lenses (they can even provide focus at up to three distances: far, intermediate and near), allowing us to correct presbyopia and any other associated prescription defect such as myopia, hyperopia or astigmatism.

With this intervention, the lens is removed, which has already lost or is losing its ability to focus closely, and is replaced by a multifocal intraocular lens, which acts as an artificial lens. This lens offers different fixed focus planes (far and closer distances) and it is the brain that chooses the most appropriate approach for each situation thanks to a procedure called pseudo-accommodation. In this way, patients recover a high range of vision and can perform almost all their activities without using glasses or contact lenses, from reading a book to driving. This treatment allows eliminating associated defects and avoids the future possibility of cataracts.

The treatment is ambulatory and the recovery time is very fast. During a variable period in each case (2-4 weeks) there will be an adaptation to the new optical system acquired. It is tried that the patient after his intervention has the greatest possible independence of correction and the highest quality for the distances most used in his habitual life (personalization in each case), although it is possible that in some cases and for situations in which it is required a lot of visual effort (computer for many hours, night driving ...) or a lot of precision or sharpness indicates the instillation of artificial tears or non-reflective resting glasses of zero or low graduation.
